For further clarity, we only collect information that you choose to submit to us or authorize us or others to obtain.  You may choose not to submit information to us, or to permit us or others to obtain it.  If you choose not to do so, however, we may not be able to provide the corresponding items or services to you.

 

The information that we collect from you does not qualify as “protected health information” or “PHI” as defined by the Health Insurance Portability and Accountability Act of 1996, as amended from time to time (“HIPAA”), because the personal information is (i) willingly disclosed by you or by someone authorized by you to provide the information for non-healthcare purposes; (ii) not used or disclosed by us or the Website for purposes of healthcare treatment by a healthcare provider or health plan, thus not qualifying as a use or disclosure by a “covered entity” under HIPAA; and (iii) not used by us for or on behalf of a covered entity for purposes of your healthcare or treatment services, thus not qualifying as a use or disclosure as a “business associate” under HIPAA.

 

Moreover, we remove all identifying information disclosed for goods or services purchased through the Website through use of random alphanumeric identifiers specific to you, but unidentifiable by us or any vendor with which we may contract.  You cannot access or obtain information or testing results from goods or services purchased on the Website without the assigned alphanumeric identifiers, and we cannot identify you from the assigned alphanumeric identifiers because we do not maintain identifying information that corresponds to the anonymous, assigned alphanumeric identifiers automatically generated from your purchase of goods or services through the Website.

 

Notwithstanding the fact that HIPAA does not apply to the Website, we protect your information pursuant to this privacy policy and related data security safeguards to protect information you provide from inappropriate access or use.  To the maximum extent possible, we will use anonymous or de-identified information that cannot be used to identify you or any individual.  De-identified information that does not identify, or cannot be reasonably used to identify, an individual does not qualify as PHI.

We use information that we collect about you or that you provide to us, including any personal information:

  • To present the Website and its contents to you.
  • To provide you with information, products or services that you request from us.
  • To fulfill any other purpose for which you provide it.
